Nomination details

The nomination process will open Tuesday 26 May 2026 and the submission form will close 11.59PM WST Monday 6 July 2026.

To nominate, complete the nomination form below. Not all fields are required; however, they are included to help you provide as much relevant information as possible against the criteria.

In choosing the recipient of the Heather Jones Community Award, consideration will be given to the nominees voluntary achievements and contributions to the community, people living with FASD and their families and support networks.

Who can nominate

Nominations will be accepted from individuals and organisations.

Self nominations are not accepted.

Criteria

Criteria 1: Contribution

Describe the individual and what they have done in their volunteer time to help people with FASD and/or their families?

Criteria 2: Impact

How have they demonstrated initiative and leadership to bring about positive change and/or acted as a mentor?

Criteria 3: Legacy

What are their inspiring qualities that make them an outstanding role model?

Selection process

Each nominee only needs to be nominated once to be considered for the award. The number of nominations per nominee will not be taken into consideration when deciding the award.
The selection process will be confidential.
The decision of the independent panel will be final.
Personal information about nominees and nominators will only be used for the purposes of assessing and judging the nominations and will not be shared.
